Print the form, fill it out and send it, along with a $15 filing fee, to Manatee County Clerk of Circuit Court, Attn: Clerk of the Value Adjustment Board, 1010 Manatee Ave. W., Bradenton, FL 34205, before 5 p.m. Sept. 20.
